WHAT ARE FLAT TIP SURGICAL LASER FIBERS:

Flat tip surgical fibers are standard surgical laser fibers with a bare flat tip designed for frontal laser emission in general surgical applications like lithotripsy and soft tissue procedures. While cost-effective for many uses, their sharp, flat tip can damage the delicate channels of flexible endoscopes, requiring careful handling and limiting their use in highly angled or deflected channels. ​

  • Flat Tip: The end of the fiber is cleaved, creating a bare tip that emits laser light in a forward, concentrated beam.

How they work
  • The fiber is inserted through a surgical scope's working channel to reach the target tissue.
  • Laser light is transmitted down the fiber and emitted from the flat tip to cut, ablate, or coagulate tissue. 
Advantages
  • Versatility: Suitable for a wide range of surgical applications, including lithotripsy (breaking up kidney stones) and soft tissue surgery. 
  • Cost-Effective: Generally, more affordable than specialized fiber tips.

Disadvantages
  • Risk of Damage: The flat, potentially sharp tip can scratch or perforate the internal channels of flexible endoscopes, especially when the scope is deflected. 
  • Limited Deflection: Should not easily pass through highly deflected channels or repeated re-insertion into a bent scope without the risk of damage.

WHAT DO FLAT TIP SURGICAL LASER FIBERS LASER FIBERS LOOK LIKE:

A flat-tip surgical fiber is a medical optical fiber with a bare and smooth end. Unlike other fibers with rounded or conical tips, the flat, straight-cut tip is designed for the direct, frontal emission of laser energy. 

Key visual features
  • A straight, flat end: The most defining feature is the perfectly flat, smooth-cleaved end of the fiber. It is bare, meaning it is not covered by a lens or other optical element.
  • Visible layers: Like other surgical fibers, a flat-tip fiber has multiple layers, which may be visible on a magnified cross-section. From the inside out, these layers typically include:
    • Silica core: The glass channel that transmits the laser energy.
    • Cladding: A thin layer of glass that surrounds the core, helping to contain the laser energy and increase the fiber's strength.
    • Coating and buffer: Outer layers that provide protection from handling and the surgical environment.
  • Aiming beam: When activated, a low-power, visible aiming beam (often green) will exit the end of the fiber. With a properly cleaved flat tip, this beam will appear as a crisp, round dot. A poor cut can cause the beam to flare.
  • Diameter: The overall appearance varies depending on the fiber's core diameter, which can range from 150 to 1000 micrometers.

​How the flat tip works
The design of the flat, "cleaved" tip is essential for ensuring efficient and predictable laser-tissue interaction.
  • Directional emission: The flat surface ensures that the laser energy emits straight out from the tip in a collimated, or parallel, beam. This provides surgeons with a predictable, forward-firing effect for cutting, vaporizing, or coagulating tissue.
  • Tip deterioration: While the tip starts out perfectly flat, it will degrade over the course of a procedure due to the intense energy and heat. This effect, known as "burnback," causes the tip to become less efficient and may require the surgeon to re-cleave the fiber.
  • Contrast with other tips: This contrasts with other fiber designs, like ball-tip fibers, which are intentionally shaped to change the emission pattern.
